X12 837 Diagnosis (HI) Validator

Accepts a full 837 or bare HI/CLM lines. Checks qualifier order, code shape, decimals and duplicates.

Clinical safety notice: Zero real patient data (PHI/PII) may be entered into this browser utility. Use synthetic test payloads only. Local-first deterministic processing; nothing leaves your browser.

How it works

Each HI element is a composite qualifier:code. In a claim, the first diagnosis must carry ABK (ICD-10-CM principal) and later ICD-10-CM diagnoses ABF; ABK may not repeat. ICD-10-CM codes must be uppercase, 3 to 7 characters in the shape letter, digit, digit or A or B, then up to four letters or digits, and must not contain a decimal point because X12 omits it (E119, not E11.9). ICD-9-CM qualifiers (BK, BF, BJ) are accepted with a numeric or V/E shape but warned, since ICD-9-CM was replaced by ICD-10-CM for service on or after 2015-10-01. A repeated code within a claim is an error, and a single HI segment may hold at most 12 diagnoses. Other known qualifiers (ABJ admitting, APR reason for visit, ABN external cause) are shape-checked only. This checks format, not whether a code exists or is billable. Each CLM starts a new claim; without CLM lines all HI segments are treated as one claim.
